I hereby apply for membership to play on the Great Lakes Tour.

The term of this membership shall begin on January 1st, 2009 to December 31st, 2009.

 

Waiver of Liability

I ACKNOWLEDGE THAT PARTICIPATION IN TOURNAMENT PLAY SHALL BE ENTIRELY AT THE SOLE RISK OF THE MYSELF. I, HEREBY REMISE, RELEASE AND DISCHARGE THE Great Lakes Tour AND ITS OFFICERS, DIRECTORS, EMPLOYEES AND REPRESENTATIVES OF AND FROM ALL MANNER OF ACTIONS, CAUSES OF ACTION, SUITS, LIABILITIES, DAMAGES, COSTS, CLAIMS AND DEMANDS WHATSOEVER WHICH I, HEREAFTER HAVE AGAINST ANY OF THEM ARISING OUT OF ANY CAUSE, MATTER OF THING ARISING OUT OF THE INVOLVEMENT AND MY PARTICIPATION IN THE TOURNAMENTS INCLUDING BODILY INJURY, DEATH, PROPERTY DAMAGE OR OTHER AND WHETHER CAUSED BY NEGLIGENCE OF OTHER MEMBERS OR OF ANY OF THE Great Lakes Tour AND ITS OFFICERS, DIRECTORS, EMPLOYEES AND REPRESENTATIVES.

 

I agree to follow the membership and tournament rules and procedures of the Great Lakes Tour as established by Great Lakes Tour (“GLT”) from time to time. I acknowledge that this application is subject to the approval of GLT in its sole discretion, provided, however, that all applications shall be accepted and consideration given for membership in the Great Lakes Tour without regard to race, religion, color, marital status, gender, ancestry, or national origin.

 

This agreement shall be effective on the date of acceptance by GLT. Membership in the Great Lakes Tour is non-refundable and non-transferable. I understand that my membership privileges may be involuntarily suspended or terminated by GLT for violations of GLT’s code of conduct or for not abiding by GLT’s established rules and procedures. This agreement shall be governed in accordance with provincial and federal laws.

Tour Policies and Procedures

Entry Information: Tournaments of the Great Lakes Tour are open to all golfers whose entry applications are approved by Great Lakes Tour ("GLT"). To apply for entry a player must: (i) mail a completed Membership Application for Entry to GLT; (ii) fax a completed Membership Application for Entry to GLT; (iii) register on-line through the Great Lakes Tour website; (iv) E-mail an entry request to GLT; and pay entry fee by personal check, visa, mastercard, certified check, money order. Entries must be received and payable one week prior to the tournament. Some tournaments may have a limitation on the number of entries. Entries will then be accepted and approved or rejected in the order received. GLT will do its best to accommodate all tee time requests made prior to the entry deadline.

Cancellations: There is no penalty for canceling an entry at least 7 days prior to the tournament in writing. Thereafter, unless a canceling player has a verified medical condition or other acceptable reason, GLT will charge a minimum penalty of $50.00 and a maximum of the full entry fee depending on GLT's obligations to the course. A "no show" on the day of the tournament will be charged a fixed penalty of the full entry fee.

Tournament Format: The format of each tournament on the Great Lakes Tour will be stroke play, gross competition. Tournament play will be governed by the current Rules of Golf as approved by the Royal Canadian Golf Association (RCGA) and Information Sheet prepared by the Great Lakes Tour Tournament Committee and by supplemental instructions from the starter.

Check-In and Starting: All tournament players must personally check-in at least 20 minutes prior to their assigned tee times and be at their starting point at least 5 minutes prior to their assigned tee time. Failure to do so may result in a player forfeiting his/her tee time. If a player is not at the starting point, ready to play, at the assigned starting time, he/she is in a breach of Rule 6-3 (penalty normally would be disqualification from the tournament). However, the Great Lakes Tour at its sole discretion may attempt to accommodate such player with a later starting time and with an assessment of a two-stroke penalty.

Seniors' and Women's Flights: The Great Lakes Tour will hold separate flight competitions for senior men (must be 50 years old) and/or for women in the Open Division if 10 or more such players are registered by the entry deadline. If there are less than 10 entries, the players will be part of the regular Open Division competition and will play from more forward tee positions set on a course by course basis.

Tee Times: It is the player’s responsibility to find out his/her tee time. The Great Lakes Tour will post the Tournament Tee Times on the Web Site at least 24 hours prior to Tournament or players may call the Tour Office.

A 2009 Tour Hard Card containing Tour rules and procedures will be presented to each member at their first tournament event.
